For example, the plan may allocate $100 for an office visit. The office visit may actually cost $120. In this case, you would pay $20 for the office visit. If you went to a doctor in their Network and the discounted rate for the office visit is $100, your cost would then be $0. The plan allocates set amounts that the plan will pay for office visits, labs, x-rays, surgery, various procedures and more. Generally, you can see any doctor, however staying with the plan's Provider Network will you cost less.
